Patient Portal In Eugene

CareToMe is Pure Life Chiropractic's Patient Portal through Vericle. At our Eugene chiropractic clinic, we want to ensure you have all the tools necessary to make your care successful. One important tool is access to Care To Me, your patient portal. Here you'll be able to:
- View upcoming patient appointments
- Update your personal and contact information
- Change your password
- Complete documents (update intake)
- View your current statement, payments, and balance
- View x-rays and exercises
When you begin care with us, you'll complete an initial patient intake, with your health history details, personal and demographic information, current complaints in your body as well as your goals - so the Doctor has all the information they need to understand what's going on, create a treatment plan to meet your goals and take the best care of you as possible.
You'll update this intake with every check-in with the doctor to ensure we have your correct information and to measure your progress. Here's how below, please let us know if you have any questions!
Before your first visit at Pure Life Chiropractic, we ask that you complete a full patient intake before your appointment.
Instructions On How To Complete Your Initial Patient Intake Through CareToMe In Eugene
- Click on the email that was sent to access CareToMe which has your unique login credentials. If you can't find this in your inbox, let us know and we will confirm your email is accurate and resend it.
- Click on the Blue Box in the body of the email, use your 10-digit phone number (no dashes or spaces) as a temporary password to log in, and then create a new password.
- Once you're logged in, you'll select "My Care Documents" (on the left-hand side).
- Click on "Create a New Document", then "Patient Forms".
- Complete all sections and sign to complete.
Before a progress report or re-exam, we ask that you update your intake before your appointment or come in 15 minutes early to complete it in the office.
How To Update Your Patient Intake Through CareToMe
- Visit CareToMe, or click on the email that was sent to access CareToMe. If you can't find this in your inbox, let us know and we will confirm your email and resend it.
- Click on the Blue Box in the body of the email and log in. If you can't remember your password, let me know and I can reset it for you to your 10-digit phone number.
- Once you're logged in, you'll select "My Care Documents" (on the left-hand side).
- Click on "Create a New Document", then "Patient Forms".
- Your last intake will repopulate, review to ensure everything is correct, update the complaints to how your body is feeling today, and sign to complete.
